The following forms and information pages available to you.

	Travel Expense Reimbursement Voucher: University Supervisors: be sure to submit this at the end of each month. In April, please submit projected expenses from April 15 through the end of the semester.
	University Supervisor Log. This form is available as an MS “e” form with fields that you can either click or type in notes or names (See Directions for E-Forms). It is also available as a form you can print off to use as a blank form.
	Teacher Candidates' Dispositions and Professional Characteristics (MS Word Document). Submit for the mid-term and final evaluations.
	Evaluation of Student Teacher by School Administrator (printed copy only). This can be used by an administrator who chooses to observe and evaluate an intern.
